Impact of a new picu on the retrieval timings of local critically ill children and comparison to an international cohort

Abstract

Introduction: A new Paediatric critical care unit (SCUH PICU) opened in the Sunshine Coast, in 2018 serving five local hospitals in Queensland. Retrievals are provided by the Brisbane-based CHQ retrieval service (CHQRS). Objectives/Aims: To characterise the impact of SCUH PICU on retrieval timings for the local population of critically ill children and comparison to an international cohort. Methods: Retrospective analysis of CHQRS database and ANZPIC registry, examining retrieval data of patients transferred from 5 local hospitals to SCUH PICU in two eras: Jan 2015 to Feb 2018 (pre-SCUH) and Feb 2018 to Sept 2018 (post SCUH). Data examined included time from referral to the team arriving at the bedside, stabilisation time (referral time to leaving the referral hospital), time to arrival in PICU and total retrieval time (team back at base). Time in hours (h). Results: One-way distance 105-170 km. There were 275 retrievals in era 1 and 45 in era 2. Median time to PICU was reduced from 6.5 h in era 1 (IQR 5.56 –7.5h) to 4.25 h in era 2 (IQR 3.75 – 6.1) p=0.0003. Total retrieval time was increased from 6.9h (IQR 6.0 -7.7 h) to 8.5h in era 2 (IQR 6.6-9.5) p=0083. Median time to bedside was unchanged; 2.4h (IQR 2-2.8 h) and 2.44 h (IQR 2.1-3 h) p=0.49. Median time to stabilisation was unchanged; 3.6h (IQR 3.0 – 3.3h) and 3.3h (IQR 2.9 – 3.8) p= 0.18 Conclusion: A local PICU has significantly reduced the time for local patients to be admitted to PICU. Time to PICU admission has reduced to less than the 260 minutes associated with improved mortality in a large UK cohort (67% < 50km). However, the opening of SCUH PICU has increased the total retrieval time (referral - team back at base); this has important implications for resource availability and service model planning.
