Cultivating compassionate environments for voice-hearers: A qualitative exploration of consumer and practitioner perspectives to facilitate care with compassion

Abstract
Objectives: Carers play a vital role in supporting people experiencing voice-hearing. This is during treatment, towards recovery, and in maintaining wellbeing. Compassion based approaches recognize the importance of these social relationships and the social environment in supporting the development of compassion, which is recognized as an important contributor to mental health outcomes. This study explored the common themes identified by key stakeholders that may facilitate carers in cultivating compassionate environments for voice-hearers.
Method: Semi-structured interviews were conducted with compassion based health practitioners, voice-hearers and their carers. The interviews with health practitioners also informed the interview schedule for those with lived experience. The interviews were analyzed using thematic analysis.
Results: The carer taking care of self was found to be important for compassionate care-giving through the interaction with developing attributes to engage with suffering, as well as the skills for prevention and alleviation. Compassionate care fostered autonomy and was associated with connection, safeness, self-worth and calmness as consistent with dimensions of self-compassion. When met with a barrier to compassion, however, the analysis indicated that suffering could be exacerbated for both carer and voice-hearer. Implications for program development and service delivery were also reported, such as collaboratively working with carers in developing truly compassionate contexts as defined in this study.
Conclusion: Voice-hearers and their carers face a myriad of challenges that contribute to suffering. This paper identified the key themes of compassion unique to this context that influence the well-being of voice-hearers and their carers.
