Family Involvement in Bedside Care in An Adult Intensive Care Unit – An Exploration of Multidisciplinary Perspective

Abstract
The practice of patient and family centered care (PFCC) in the intensive care unit (ICU) has many benefits to both patients and their families. However, evidence indicates that practice varies across the globe especially in adult ICUs with limited research focusing on family involvement in care at the bedside. Furthermore, little is known about family involvement in ICU care from perspectives of clinicians other than nurses. The study aimed to explore the multidisciplinary perspectives on involving families in bedside care at an Australian adult ICU, including their perceptions of the concept, experience and perceived barriers and enablers. This study adopted a qualitative research approach using interpretative phenomenological analysis (IPA). Semi-structured in-depth interviews were conducted among a purposive sample of clinicians working in an adult ICU within a Western Australian tertiary hospital. Descriptive statistics including mean, median and percentage were produced to summarise the sample characteristics. Interview data were analysed following steps of the IPA process. Six clinicians participated the interviews: three from allied health, two medical professionals and one nurse. Data analysis generated five themes which described these multidisciplinary clinicians’ perceptions and experience of involving families in bedside care within the ICU: The meaning of involvement; Perceived benefits of involvement; Perceived risks of involvement; Factors influencing involvement; and Decisionmaking regarding involvement. This study shed light on the involvement of family in bedside care within the adult ICU from the perspectives of multidisciplinary clinicians. Findings revealed the complexity of decision-making in which staff sought to balance perceived benefits and risks of such involvement taking into account various factors related to patients, families, clinicians and the organisation. The findings provide directions to develop education and training, policies and practice guidelines, as well as family-friendly environmental design to facilitate PFCC in adult ICU settings.
