Abstract
With such an ostentatious title, this 2024 drug allergy theme issue in The Journal of Allergy and Clinical Immunology: In Practice again promises a series of high-quality and exciting articles that combine a review of existing knowledge with new innovation. But first, why would we look beyond the classical and established paradigms? Clinical practice and recent research have highlighted that much can be learned from and done for patients with reactions that do not fit under the classical “drug allergy” umbrella. This is either because they do not have a true drug allergy or their response mechanistically fits outside the existing classification paradigm (Figure 1). More recently, Jutel et al1 have attempted to reclassify hypersensitivity reactions by extending these into antibody (I-III), cell-mediated (IVa-c), tissue-driven, and direct cellular and inflammatory responses to chemicals (VII).
