Chapter Highlights •This chapter presents an overview of anesthetic considerations relating to upper airway surgery for patients with obstructive sleep apnea. •Insightful anesthetic management of such cases requires an appreciation of the similarities and differences between the sleep and anesthetic states in relationship to their effects on upper airway and breathing function and on the arousal responses that help protect against disturbance of these essential physiologic functions. •These issues are discussed in regard to both sedation for preoperative evaluation for obstructive sleep apnea surgery and perioperative management when surgery is undertaken.
