Obstructive sleep apnoea (OSA) is a highly prevalent condition that, when left untreated, is associated with significant adverse health consequences.1 Growing recognition of the substantial global burden of undiagnosed OSA and limited access to specialist sleep services and laboratory-based sleep study testing has led to increasing interest in the development of simplified diagnostic tools and management pathways for OSA based within the primary care setting.2 In a recent publication in Respirology entitled Development and validation of a model for diagnosis of obstructive sleep apnoea in primary care, Chapman et al.3 evaluate a diagnostic model for OSA for use in primary care by assessing the utility of OSA screening questionnaires, anthropometric measurements, digital facial photography and a single-channel nasal flow monitor. From their study findings, the authors conclude that primary care physicians (PCPs) (i) can identify most patients at high risk of OSA based only on clinical impression, (ii) need the additional measurement of nasal airflow to rule-in OSA or rule-out severe OSA and (iii) do not need to use screening questionnaires or other basic clinical data as they perform poorly when trying to rule out OSA...
